Financial forms

Jockey and Trainer Financial Details Form

Use this form to provide or update taxation, GST and bank account details if you are a thoroughbred jockey or trainer.

Return the signed form with a copy of your driver licence or another primary form of photographic identification to Racing Queensland. Email the completed documents to [email protected] or fax them to (07) 3269 6404.

Change of Prizemoney Details Form - Jockey, Trainer and Owner

Use this form to update the bank account used to receive prizemoney if you are a jockey, trainer or owner.

Prizemoney payments to a new bank account will begin no later than 5 business days after RQ receives the fully completed form and all required information, in accordance with the form’s terms and conditions.
